My folder_format seems to be ignored for my imap folders:
set folder_format="%d %N %f"
Nothing at all shows left of the folder name.
What I have set in muttrc:
set spoolfile=imaps://domain.net
set folder=imaps://domain.net
mailboxes +=imaps://domain.net
mailboxes +=INBOX
etc

I think that POP-before-SMTP and IMAP-before-SMTP
should work just fine
