The patch should apply against the tip of the default branch. It may
take a little bit of wiggling to get it to apply to something older
because of changes
I'm using mutt 1.5.21 on OS X to read mail from Fastmail IMAP. I have header
caching enabled and set up, and as far as I can tell, it is
being used. At least, all the .hcache files have recent modification times.
