I've tried incorporating GTD with Evernote which worked pretty good but MLO
does it way better.
What I would love to see in MLO 4 is the default status "Not Started" for

that have 2 contexts. For example I'd like to have a task that had 1

If you are looking for a workaround, you probably can get around this by
creating a custom view, if you only want to see Projects that are not
started rather than all tasks.

Yes once I have decided on what I want to do - but what I would also like is
a view which I use when reviewing priorities/planning which shows my chosen
'priority' contexts where I can drag and drop items between the different
contexts as I think about what is most important for me to working on.
In MLO 3, this would have required a single view with the relevant contexts.
MLO4, now in beta, should enable you to address this with a view for each
context, each view open in its own tab, using drag & drop to move tasks from
one view to the next.

Your request at face value appears impossible as the open hours "include
closed" do not seem to associate with a field in the advanced filter. I
can't think of an easy workaround.

I *believe* that reminders remember the last setting you used. Therefore
if you choose the "reminders window" on the last task instead of an email,
the next task you set will use the "reminders window".

headings/groups where I can drag items from one groups to another. Eg at

Richard, I think you were pretty clear about what you are looking for.
Unfortunately, I do not think you can get what you want, the way you want
it, without significant changes to MLO. When you have a view that groups
tasks by context, you get a context heading for every context that is
present on
BOC: I'm not sure if this is what you were looking for, but in the advanced
filter for Context there are two versions of "contains" - one considers all
contexts and the other considers only open contexts.
