SHOW INDEX FROM tablename
Should let you know what columns make up the MUL column INDEX.
If this was not supposed to be indexed, your table is corrupt, best try

What about upgrading to MySQL 4.0.13 ?
4.0.0 is not a "production" release, and a lot of bugs have been fixed
between 4.0.0 and 4.0.13.
