I'm getting the recovery notifications as long as the problems aren't
acknowledged, but once it's acknowledged, no recovery notification.
Any other ideas?

If you want acknowledgment to disable notifications until the service
recovers, check the 'Sticky Acknowledgment' checkbox.
It looks like any kind of acknowledgment disables notifications.
