I used DarwinPorts (now MacPorts) to install my Nagios setup. I see NRPE 2.5.2 available in the repository.

Read the compiling documentation, if there is no Mac
OS X documentation, then follow the FreeBSD / *BSD advice

Compiling stuff on the Mac shouldn't be any different from other
Unices, in general.
