Which model HP Server?
I haven't seen this on my DL models ( 360-380, 580) or ML models.
Using latest drivers/Firmware? Also which patch is applied?

It's a mish mash of HP kit, G3 - G5, and in terms of the patches
yesterday SP2 did it twice, few weeks back an update to the MPIO.sys
driver on both nodes in a cluster did it
