Can you run kdebugdialog, write okular in the search line edit, enable all the
okular debug, then run okular in a terminal, open the file that shows this
problem and attach as file all the output

I tried with the attached file but I couldn't reproduce that. Could you
double check your okular version? Is it 0.16.1?
