 1.  Create and send an email with the  "To"  address of:   HMIS HelpDesk
Trouble Ticket Mail/HMHP/CHP
 2.  Include a call back number so our staff can contact you, a description
of the problem along with the support team or individual the support ticket
should be assigned to if that is known.
 3.  You will receive a Confirmation Email back stating the Dk Ticket
Number for tracking and reference purposes.
