OK - I think I finally have the issue nailed down... Curently, /usr/sbin/suexec is owned by root:root. Just for the heck of it, I changed it to root:apache, and now otrs works great.  However, everything else on my webserver that runs cgi no longer works.  So I changed the ownsership of suexec
