Actually it's a known bug in OTRS:
http://bugs.otrs.org/show_bug.cgi?id=4658 - you can subscribe to the
bugzilla to be kept up-to-date with the progress on that one.
You can however set attachments to "Invalid", or re-use them, as a
workaround in the mean time.
