We work as following:
We get a ticket request.
If we don't know the answer, we forward this question to our supplier.
We receive an answer from our supplier.
We forward this to the customer.
This works fine in most cases.

You're right, if you're running OTRS as CGI, this issue will occur. If
you happen to run OTRS with mod_perl (as you can with the mod_perl
installer from our website) this will not be a problem.
