The reason is there is a bug in MySQL->Apache that causes your DB threads to go to sleep and never wake up. It will work for about 5 minutes then stop responding. Looking at Sambar or a Linux box solution might be a better use of your time. I found the SuSE 8.x install of OTRS to be downright simple.
