Look in Apache's access_log and error_log. What do they say?

I think there is an option where you can set the path of this file in
the Kernel/Config.pm.
Plz have a look in it.
Or the other way is that you create this file.

error.log file. these errors were logged right from the start of Apache
server till i tried to add a new User into the system.
