reg papers regarding the date, Hospital, wt and MW, we also fill
out the centerlink form and give it all to the woman with the
baby's personal health record (Qld),Community health and sids
stuff and various other educational
I was at a birth recently and after she had the baby the mother asked me about the paperwork and whether she could get a registration reissued. It turned out that she had never registered the previous child (now 4 years old) and had lost the papers! We were able to reissue the registration papers
