You might want to take a look at PhpAdsNew, a banner rotation/management
system using PHP and MySQL:

The system's admin tools assume that you're selling ad space, but there's no
reason you couldn't use it for a banner exchange instead.

Based on the GET_VARS you can identify the image and do some
mapping stuffs to get the sitename. Then you can construct your header
accordingly with 'Location:' parameter to view that site.
