I figured out that I was running an older version of Suhosin. Apparently
this bug is fixed in Suhosin version 0.9.23 . I upgraded and everything

I think that the safe mode is on
But no executables into the safe_mode_exec_dir
beware if you set the safe mode to OFF. This is my value, but my server
is in a safe place.

fopen() with let's say 'a' will open the file, and put
the cursor to the very last byte. The time period
between opening the file and getting the lock is
probably where you're clobbered.
