Internatinalize index title and put it to an appendix so it
will integrate quite nicely (will get an appendix label and
a separate chunk in XSLT transformations).
Following are the top 20 pages of the manual, sorted by the number
of user notes contributed. These sections could use a polish, those
notes represent 14.6% of the 7311 total user notes.
