In light of the a recent bug with the run time call by reference
examples. I started going through and finding the examples that
have used the &$var syntax.
I was thinking, perhaps the proto should reflect that that
the parameter is passed by reference:

string by accessing an individual character in the string with the index
operator (curly braces) and an assignment statement, as in:
$str = '0123456789';
$str{4} = 'd';
