I have made some modifications on revcheck script.
It is now in conformity with the xhtml 1.0 strict specification (semantics is

No, you should revert it
So, using count() in a for is bad, but okay in a foreach ?
foreach makes a copy, for and each work directly on the subject.
the reason you were told to evaluate the count into a tmp var was to
save the loop from re-evaluating a constant (in most cases, for the
duration of the loop) value.

Docs,
The apos; entity in the wrappers appendix causes problems with
Livedocs. (e.g. http://docs.php.net/wrappers)
This is because html_entity_decode(...) doesn't understand apos;
According to the w3c, apos; is valid (') (MAY be used)
Philip just pointed out to me that if we tell html_entity_decode() to
use ENT_QUOTES, this should work.
I'll test submit a patch for livedocs.

Is still showing a bullshit example. It constructs a destination path
for move_uploaded_file() that consists of user input. There is no sanity
check on it like removing / and .. sequences.
