Werner has previously fixed up some of the D examples and also today added
some new ones so I thought it would be good to review the D status again.

Example 14 produces errors when used in the test_d.sh script to generate
PostScript results so I had to exclude it, but otherwise I have been able
to add all implemented D examples to test_d.sh.in (revision 10006).  Here
is the resulting summary results for D (done with the new CMake-based build
system for the installed examples tree and "make test_noninteractive").

Although there is obviously quite a bit of work left to do to perfect these
D bindings and examples this is a considerable improvement over the last D
