One issue: All the notifications using FoxNotify that are triggered from
FoxForum are bearing a URL to "/Forum/NameFromFilter"
Also, the [[#default]] section follows the [[#heading]] on the same
line; I am unable to add any spaces or linebreaks.

Try this:
change the entry in FoxNotifyLists.ListA to
notify=emailaddres format=#comment
Since you use a field 'comment' the value will be used by the email
notification and inserted into the email body.
This is determined by FoxNotifyLists.FoxNotifyTemplates
