I am sorry but can you tell me where to add the append. I want to
have all of the data appended the same page with the image. I tried
to add append but I only get a confirmation that the image was
uploaded and nothing else...

It also looks like you don't have an append or prepend
entry on this page. Since you don't have a target
listed here:
(:adl form entry:)
it will look to add the text to the current page and
if the append/prepend tags are not there, it will do
nothing,
