before I read your message yesterday I deinstalled 2.18 and compiled and
installed 2.99 from AUR, again with the option -DWITH_CUSTOM_WIDGETS=TRUE.
And it worked. In this case Qt5 Designer contained the Qgs custom-widgets.

Can you look in the about menu, there is a plugin entry, see if qgis custom
widgets are listed there as broken.
If so, can you try
ldd /lib64/qt4/plugins/designerlibqgis_customwidgets.so

Designer plugins need to be installed to the proper location, which
unfortunately isn't always straightforward and to my knowledge needs to
be manually configured by packagers.
The important thing is, that the libqgis_customwidgets.so file is in the
location where QtDesigner looks for it

I am trying to work with the Qgs custom-widgets in Qt4 Designer. I am
running Manjaro on kernel 4.14.
So far I tried to install QGIS 2.18 from the AUR compiling it with the
option -WITH_CUSTOM_WIDGETS=TRUE.
No custom widgets appeared after that in QT4 designer.
