p. 122-125 - you need to know the coordinates of individual features (3-5
per map sheet) of the target coordinate system.

Can you connect from other machines with other software (e.g. pgadmin3)
using the same settings?

QGIS uses a network socket connection whether or not it is on a local host, so
Postgres needs to enable such connections.

I think it's worthwhile asking in the user list, users are the ones
who must decide if a given functionality is needed or not. Then
developers decide if this is possible or not (and if they want to do it
or not!).
And yes, a set of buttons as you describe would be useful.
