[qmailtoaster] some mails get lost between qmail-smtp and qmail-send

I have problems with specific user/domain. There are traces in the log that mail has been accepted, but after that i don't see a trace of it in qmail-send (and of course mail does not get delivered). In very rare occasions it get's delivered ( same situation, same sender smtp) Where should I look
