You mention that some users are connected fine -- any chance you're

Don't know about proxies, but message duplication has been caused by
courier-imap, which can be fixed by switching to dovecot instead

Is it the same error in the Dovecot log when trying to connect with Outlook?
Is this only one user who's experiencing this problem?
Is this user always trying to connect from the same computer?

We don't face this issue on other email client except squirrelmail.
Outlook and thunderbird email client are working perfect.
but if i try via webmail i got this error for all the users in different
computer.

If i do downgrade php is it affect any qmailtoaster and other email clients
or dependency.
What is steps to downgrade and test it.

I can't be sure that php is the problem, the question is, did
squirrelmail work fine before the upgrade from php-5.3 to php-5.5? That
said, I think that the only QMT rolled rpm that depends on php is
squirrelmail, but I'm not positive about that either. I think you can
check this using
