However, can someone please explain why this MAP! line got into the log
file?

suppose if I only used it with
a single RBL using A records, I could change the $result text to
whatever I want and be done with it.
And it doesn't look like there is a reverse RBL function. Am I
making the right assumptions?

qpsmtpd plugin, I have no idea what its status is
when it comes to 2.61. A month ago it did not work.
So I presume, then, that 2.61 works with your plugin. I'll check

If this is to suggest that you drop the connection unless the test
is accepted, I see a problem: Some MTAs will accept anything, and then
reject or bounce later. I don't see the extra traffic and delays as worth
it.
