I am disabled and have been using DragonDictate {now NaturallySpeaking}
since I first got her back in 1993. As far as I can see anything that you
could do with DragonDictate you can still do with NaturallySpeaking. It
still has the MouseGrid command that was there in the earlier versions of
Dragon

Your choice of hardware is going to depend on your ability to
move. I use a quadjoy mouse. If all you can do is move your
eyes, they make eye gaze systems that track your eye movement.
If you just have gross arm movement, try the bigtrack. I used it
