Hello Debra, It's hard to advise more specifically without knowing what state and city you live in, but most if not all states have regional Area Agencies on Aging (AAAs) and most of them provide an Aging and Disability Resource Connection (ADRC) or some similar service that you should be able to contact and see what services, waivers, etc. you may be eligible for to transition from the nursing home to your community. These waiver programs and services may have waiting lists, some lengthy, so it's best to apply for and be added to any existing list for eligible support ASAP.
I also encourage you to contact your local Center for Independent Living (CIL). Like the AAAs and ADRCs, most if not all states have regional CILs, each serving multiple counties. Many CILs have staff and/or programs dedicated to transitioning consumers from nursing homes to the community or helping to prevent them from going to a nursing home for long-term care.
