You can make a `toc-target-element` instance to add to the "on this
page" content to the left of a documentation page. I think you'll have
to use `scribble/core` for that, since `scribble/base` doesn't provide
a wrapper for it (as far as I can remember).

Use `make-screen-bitmap` to create the destination bitmap, and draw the
pict into that bitmap with `draw-pict`.
I think `pict->bitmap` should take an optional `#:make-bitmap`
argument, and then you could provide `make-screen-bitmap` for that
argument. I'll make that change.
