Looking at this again, it is more likey because the radius reply is coming back from a different IP address (or different port number) than that to which the request was sent.
Check the trace 4 debug on both hosts, and if this is the problem you can use the following:

I suspect your Client clauses on remote-ip end are not set up correctly.
Have a look at the trace 4 debug on the remote-ip end to see what the exact problem is.
If you still have problems, please send me copies of the trace 4 debugs from both hosts.
