Last time I was at Rivendell, I bought the Yokozunas they sell in both smooth and threaded (I needed threaded). Their spacers and cupped washers were really easy to set up and adjust. When I was there, I noticed all the brake pads were toed in to the front to keep them from squealing which is

this is all I use on all my bikes. If they were too long as on one bike I just dremeled it off at the first ridge then you can go ridge by ridge as needed. A waste of good pad but squealing brakes are the worst!
