Solved! The screw on the barend shifter came loose and the components were
slightly out of alignment making it so I could not get a full throw of the
shifter.
FWIW, a long cage 9 speed deore rear derailleur will shift at least up to a 42t
cog with the stock b screw full tightened.

Conway, If the screw doesn't work out, one of those Wolf Roadlink derailleur
tab extensions would also work. There's many brands of them but only the Wolf
and Wheels Mfg. ones use a steel screw that I know of.
