That is why you're getting the crash.
People aren't directly, actively accessing it, but I bet more than one is
on the Dashboard, and that periodically reloads.

This sounds like the installation once again partially failed. The list of
repository types is populated during install, and should be available in
that dropdown, but that may not be happening.

when I post a review request I do not get any default reviewer added
to my review request.
