It appears that post-review does not support Bazaar yet.
There is an issue for this in this tracker:

At work, we only use post-review from command line, so I have no
experience in using GUI, but I'll try to test it tommorow :-)

I checked permissions - it turned out that ext had permissions of 755 and
infact needs 775 as the user is in group www-data but the www-data needs w
access to ext.
Perhaps that could be added to the debug message to help other people who
make the same mistake as me.

below error page when i try to create a new review request in GUI

If i try to upoad-diff on an already available i get the attached error.
