You can use NTFSDos to grab the SAM file under NT.
Boot you machine with a normal bootdisk and then use NTFSDos to access the
drives.

I once heard about the possibility to hex-edit some file and change the
information this way.
This can be done on apache webservers and a good example is the server at
hackerslab.org.
Try to search through the IIS files and then hex-edit them.
