Nope. Just some headers may appear multiple times. Have a look at
Section 7.3 of RFC3261 where the BNF pattern for those headers that
can occur multiple times (as comma separated list or as multiple
headers) is described. Use this pattern when consulting the ABNF.

In terms of ABNF in RFC 3261, headers can appear any number of times.
However, in practice, it only makes sense for some headers to appear multiple
times (e.g. Route, Record-Route, Contact).
AFAIK, multiple To, From headers in a SIP message have no use (not at the
moment atleast)

There are a number of headers that may only appear once, including From,
To, CSeq. IIRC this is addressed in the text relevant to each one.

A SIP proxy is not a UAS, it's a proxy :)
A registrar server is a UAS.

An UPDATE is an UPDATE. The effect is strictly a function of its
content. And as already mentioned, every UPDATE either refreshes the
session timer, or disables it, regardless of what else it does.
