We reinstalled a 3.10 system with 4.0 from ISO, and upgraded to 4.01. In both 4.0/4.01 we started seeing issues with CDR's for calls showing "Failed". The only calls marked successful were calls to internal users, media server, etc. We are using sip trunking with an Ingate as an unmanaged gateway
