What you are describing is typically how phones and PCs are connected in
an enterprise LAN to do VoIP. You need to program your phones to
connect to the VoIP VLAN using VLAN2 so all traffic generated by the
phone will be tagged with VLAN2. The phone will switch all traffic
received on its PC

It was indeed the case that the INVITE sent by the Cisco gateway in
response to the REFER did not contain the X-Sipx-Authidentity SIP URI
parameter. The gateway removed that parameter when it generated the
INVITE based on the Refer-To header in the REFER message.
