I have the same problem but found if you manually disconnect the ADSL line from
the modem, at the socket or the cable junction, then try modem_run again it
should work.
not elegant but avoids a reboot.
BTW: not using latest cvs and Os is RH7.1 2.4.9

Before you buy another card, I wouldn't think this is your problem (although
trying another card shouldn't harm anything ...).
My problem was occasional system lock-ups - I could connect to the Internet
ok.

If someone experience that kind of problem, the trick is to use only the
usb-uhci module with kernel newer than 2.4.10.
