It seems You're right. I've checked two kernel-source packets:
kernel-source-2.4.21-0.13mdk.i586.rpm - MDK 9.1
kernel-source-2.4.22-10mdk.i586.rpm - MDK 9.2
Both contain "custom" suffix in EXTRAVERSION.
