Hmm... probably that must-revalidate condition is giving you problems.
Seems Squid does not allow you to override things if this is set.
Can't you fix the server?

DNS server to the IP of the server and it does not want to launch. Is there
anything else I should be looking for? Or is it connecting to something and is
waiting for reply?

Good question. May be possible using the rep_header acl looking for the
X-Squid-Error header. But haven't verified.
