You can set ftp_user in squid.conf, but it's a system-wide option.

Look into X-Forwarded-For, you need to recompile squid with that option
and add the x-forwarded... lines to squid.conf

often crashes with:
FATAL: Too many queued url_rewriter requests (54 on 12)
This seems only to happen when the cache is busy. Once the FATAL has

Caching dynamic content doesn't work like that.
Firstly, removing the QUERY ACL gives you the ability to cache dynamic
content that returned explicit lifetime.
You need to look at all of those MISSes and see why Squid isn't caching
them. Its hard to tell from where I'm sitting.
