Could you try a more recent version of squid?
I don't think that 2.6S4 supports proxying content when the server
only offers ntlm authentication

Firefox will say something along the lines of the proxy isn't setup to
accept connections.

Do other clients work OK ?
Did you check if Squid crash occur (check in cache.log) ?
I there a specific scenario leading to RESET ?
Squid doesn't send RESET without reason, check for reason in capture

possible to check for a group membership during/after authentication to
allow a user to use SQUID? For instance, I want to be able to take away
or grant access to the proxy based on an AD group membership.

and samba 3.0.20b I will upgrade these proxies into 2.7.4/3.0.32 next
monday but before doing this I would like to ask you your advices
and/or
experiences with tuning these kind of boxes.

you should update part of refresh pattern and store_url_rewrite.

within the buffer limits (SQUID_TCP_SO_RCVBUF). It's not relaed to
previews.
Why is there such a limitation (SQUID_TCP_SO_RCVBUF) ?
I hope that squid always send Allow: 204 to icap servers as much
as possible...
