Our squid servers are consistently goes over their configured disk
limits. I've rm'ed the cache directories and started over several
times... yet they slowly grow to over their set limit and fill up the
filesystem.

This can be achieved through NTLM or Negotiate authentication, in which
case the browser will try to use the PC credentials to authenticate to
squid.

I don't know if there is a helper already written to do what you are looking
for, but have you looked into existing external_auth based helper programs?
