Do you have any deny lists or blocked sites? what I
found when I was setting up squid was that some of the
images would be named or come from a site i had
blocked so they wouldnt show up.

so then would proxy_auth "/usr/squid/user.name.file"
only allow exact user name matches?

Actully what I was doing worked i just had to change
how i had the user names. They needed to be in a their
own line not all in a row with white space between.
