First thing to do : find cache.log, tail it, see what it says.

A look at your cache.log
Post the relevant bits!

After check the cache.log and your squid.conf, and if
its looks ok, delete these two files:
/var/spool/squid/swap.state and swap.??clean (do not
remember the exact name of the second file, but start
whit swap)
