I'm seeing the same problem as well. It seems to
happen with greater frequency now for some reason. I
haven't changed my PHP configuration since I upgraded
to 1.4.2.

It's looking like there is something in the
configuration of Apache or PHP that's really causing
the problem.
I put 1.4.1 on another machine that I already had a
running PHP bug tracking application and brought up SM
in about 10 minutes.
