In Examples 11-13, and 15, 16 (and 14 if you meant to have the original XML there too) of http://xmpp.org/extensions/xep-0248.html , there is no field var='FORM_TYPE' type='hidden', whereas the other examples with a child x xmlns='jabber:x:data'/ in configure/ have it. If it is not to be there,
